Important things to note:

• You can enrol if you are permanently residing in New Zealand.
• You can only be enrolled with one GP provider at a time.
• You must upload proof of eligibility documents to enrol.
• You will be contacted when your enrolment has been processed and your records received from your current GP.
• Doctors at Beckford Health may be unable to prescribe until your records are received.

Proof of eligibility documents:

Please have ONE of the below documents to upload when you're ready to enrol. Documents suitable for proof of eligibility for your selected eligibility criterion are:

• NZ passport OR
• NZ certificate of identity OR
• NZ birth certificate OR
• Cook Islands, Niue or Tokelau birth certificate OR
• NZ certificate of citizenship OR
• Certified copy of a descent registration certificate OR
• Social security benefit papers (except emergency benefit) AND two forms of supporting identity information, one with a photo (unless passport is provided).

14 -17 years = $78

Community Service Card Holder

18 years or older = $50

14 -17 = $25
13 years No charge
